Smith County TN is above the threshold for COVID 19 cases per 100,000. Thus, Smith County Health and Rehab will not be able to allow visitation until the number of COVID cases in the county subsides. Thanks. Management
Smith County Health and Rehab received confirmation today that a resident who was discharged from Smith County Health and Rehab on 07/09/2020 tested positive for COVID-19. The resident was not exhibiting any symptoms and was only tested as part of the Assisted Living facility admission process. Thankfully, the resident is still not exhibiting symptoms and is expected to make a full recovery.
We have notified the Department of Health. At this point, there are no other residents or staff who have presented with any COVID-19 symptoms, and we have not had any other positive tests. Thanks

We have communication frequently with our team and are doing well during this challenging time in our nation. We completed our resident testing for Covid 19 and the labs have been sent for processing and the staff were tested today. We had a few staff to test positive from last week, but they are not in the facility. They are on quarantine until further notice. We appreciate your staying in touch and we will continue to update. As part of our transparent communication, we want to let people know the general guidelines and actions that we are taking daily at Smith County Health and Rehabilitation. We are following State and Federal guidelines, from the CDC and our local Health Department. We are utilizing the following procedures:
• Screening Table for Temperature taking, questions, signing of entrance/ exit log. We also have a shoe station to sanitize shoes
• PPE: our staff have all the resources they need, provided by Clearview, the local Health Department, and other Corporate volunteers. This includes face masks, sanitizer, gloves, gowns, goggles, face shields, etc.
• We are conducting extra cleaning and sanitizing throughout the day, this includes using BRU tabs to spray down surfaces
• We have visitor restrictions per the CDC guidelines to help mitigate the chances of COVID- 19 spread
• New admissions are all quarantined for 14 days
• Twice daily, our clinical team makes COVID-19 surveillance rounds, looking for any potential symptoms to ensure resident safety
• We have facility wide educational staff update COVID-19 meetings
• We have facility conference calls with ClearView that provide support for any of our needs
• We are using dedicated resident living areas, with dedicated staff, to also reduce potential spread
• Department Heads conduct Infection Control Rounding
• We have a separate unit, if needed, for positive resident cases
• Smith County Health and Rehab is a member of the Smith County COVID-19 Task force
• We have monthly family conference calls/ letters for updates regarding COVID-19
• Formal reporting via NHSN and SAMS regarding COVID-19 and the Smith County Community is done weekly. We encourage you to look at our website for updates. We also send formal updates to the Health Department
• Our Medical Director and Nurse Practitioners are ensuring quality resident care whether it be medications or nutrition
• Our Activities Department and staff continues to put in great effort to maintain a happy life during this challenging time
• We are doing facility wide testing for both residents and staff to measure our continued progress
Today we had approximately 10 additional staff to receive positive test results for Covid 19. In addition, 15 residents received positive test results for Covid 19. There are still quite a few pending resident test results. As soon as we get those labs, we will convey that info to the proper parties. We have standing doctor’s orders to treat residents that address various levels of care. Our staff are notifying families as we get information, test results, condition changes, etc. We greatly appreciate loved ones patience with our communications, for our first and foremost, primary focus, is quality resident care. We are continuing to follow CDC guidelines and have the full support of our company. We have access to any resources we may need. Our staff are working hard to ensure our residents have the best, loving care possible. Everyone is ok at this time, and our team is navigating this challenge like the seasoned professionals they are. We ask for your continued prayers and support. Thank you very much.
As of today, no new confirmed cases with residents or staff. We are doing testing and will reveal those results as we obtain them. We continue to follow CDC guidelines and are screening, sanitizing, and utilizing the appropriate PPE. We have dedicated areas for Covid positive residents in an effort to mitigate spread. We have 100% support from our state and local officials, and we have 100% support and resources from our company. I want to publicly thank our dedicated staff who are working hard to help get us through this challenging time. We appreciate your continued patience and support as we make our way to the other of this challenge.
We are doing a good job one day at a time. We are currently awaiting 13 resident tests. These residents are doing well but we are testing anyone, even if they have the sniffles, just to be sure we don’t miss anything. We are conducting a Facility wide staff test Thursday 7/23, like we do each Thursday. We have Facility Wide resident testing being conducted, too. We will be sure to post any updated info we receive. We want to thank our staff who are literally hero’s for working hard in a very challenging situation. They are unselfish and are sacrificing time from their personal lives and families to be here for the residents. We are proud of every single one of them!

Of the pending resident tests, three have returned positive. Our goal is zero positive results; however, this reduction in number of positive cases is very inspiring in regards to the team’s efforts to deal with, and prevent any further spread. This virus has made its way into every nook and cranny of the world. We are frustrated it found it’s way here, but we are using every tool in the CDC and Health Department’s toolbox to mitigate the spread and extinguish it from our community. We appreciate your continued patience and support during this challenging time. Our staff are working tirelessly to ensure the residents get the best care, and that we continue to kick COVID-19 to the curb.

We are doing well at this time. We are awaiting our most recent staff test results, and are testing for baseline on 2 resident hallways. Our community is still receiving large amounts of PPE from our company, the Health Department, and corporations, like Ford. Our Activities Department is doing their best to keep everyone busy. This is having to take place in a one on one manner because of the social distancing and isolation we are dealing with at this time. We have Face Time appointments that can be made with your loved ones, by contacting Leslie Sanders, our Activities Director. Each day we are screening, and sanitizing, along with surveying the residents for signs and symptoms, and following CDC guidelines to mitigate spread and help to extinguish COVID 19 from our community. We appreciate you reviewing our updates, and continued support.
Some resident and staff tests are still pending, but overall we do not have many symptomatic staff or residents in the building. Just a little more insight into the fortress we have built for Infection control, and sanitizing, we have plastic barriers to every hallway inside the corridor doors to further assist with Infection Prevention. We have fans inside each Hall to hep with potential infection spread and help to knock any potential droplets, etc. to the ground. We have hand and shoe sanitizing stations at the entrance/exit of each Hall. PPE is readily available and in use. These collective efforts appear to be stabilizing and reducing the spread in our community. I thank our heroic staff again for their continued effort to ensure the best resident care possible. We have hired additional Nurses and CNA’s to make certain the we have all the support possible. Even our Regional team members are here on most days to provide our community with their experience, and resources. I am very proud to work for ClearView. I have never worked for a company that went to such great lengths to make certain we had everything we need, and most importantly, everything the residents need. Your continued thoughts and prayers are greatly appreciated during this trying time of this Worldwide Pandemic.
We had a smooth day with everyone focused on our residents, along with a continued special focus on Sanitizing and Infection Prevention. Our efforts to mitigate appear to be fruitful, and we will keep sharing any new information. Right now we don’t have any new test results returned from the lab. Our Activities Department is continuing to do their Face Time visits with residents and their loved ones, and doing one on one room visits. We hired two more nurses over the weekend to join the team to provide even more support to our nursing team. We are celebrating the staff’s dedication and sacrifice during this Global situation. Each day we are getting them lunch and taking time to thank them personally, and let them know how much we appreciate them caring for our residents. These folks are true hero’s. Thanks.
Our staff test results are back and we had a total of 8 positive cases, as opposed to 14 in the past, so progress is being made. These staff for the most part are asymptomatic and feel fine, only a few have mild symptoms. From our resident testing, we do have 3 more residents that are positive. We want to state again that the courage of our staff is remarkable. They continue to stand tall in the face of unprecedented adversity. We have a very large sign out by Highway 25 that runs East and West beside our building. It says “Hero’s Work Here”
There couldn’t be a more accurate slogan to describe the staff that are taking care of our precious residents.
Many people from our community have shared they are praying for us and are here for anything we may need. We greatly appreciate these folk’s support. We have 100% continued support from our Company and the Regional Team. We have all the supplies and resources we want and/or need.
A few more resident tests came back positive. This is quite a bit less compared to our first testing outcomes. We do not want any new cases of course, but seeing less cases is certainly encouraging.
Again, it appears we are stabilizing the spread, and our Infection Prevention efforts are working. We continue to have New Hires join our team, which is great. We are blessed to have the added support. We have our weekly Covid-19 staff testing each Thursday morning. This means tomorrow we will test the staff who haven’t tested positive so far. Those staff who have tested positive will begin testing again after two or three months. We are saddened by those members of our community affected or harmed by the Covid-19 Virus. This Pandemic has changed the World. Everyone has had their live’s altered to some degree. We pray for everyone in our community and those across the Globe. We will persevere at Smith County. We are working hard to see the residents get all the Activities possible, and that the staff’s spirits are lifted, too, by thanking them , and being here for them in this time of need. We are very proud of our staff, who are angels of sunlight, in the midst of this storm. We appreciate you following our updates, and we appreciate your continued patience and support.
Still awaiting 4 resident tests. In the interim we are working with the State Surveyor to ensure our processes and Infection Prevention measures are working and are as robust as possible. We had zero deficiencies and were given a couple recommendations on how to streamline our screening process. We are very proud of this, because the staff work hard every day to mitigate spread and follow the CDC guidelines and go above and beyond wherever possible.
We are blessed to be able to say our efforts are working. The number of resident positives are approximately one fourth of our original testing. The majority of the original residents that tested positive have recovered and are not symptomatic. We will be soon having “I beat Covid” recognition ceremonies for residents and staff who have fought to defeat this awful Worldwide Virus that originated in China. We are solemn over the people who have been harmed by Covid-19, and our thoughts and prayers are with every person affected. On the other hand, we are trying our best to raise the spirits of those who have made it through this dreaded Virus. Our Face Book page will start having some pictures of the “I beat Covid” ceremonies we plan to have in the near future.

Our most recent testing revealed no new resident positive cases, and 4 staff positive results.
Each week our numbers are dropping. This has not happened by luck. This is happening because of the team following the rigorous CDC guidelines for Infection Prevention. These start at the front door with screening of everyone entering the facility, to the daily sanitizing of the building, and the equipment within the facility, whether it be tables and chairs or handrails. This is done around the clock to ensure the cleanest, sterile environment possible. The staff have a surplus of PPE and are trained in how to use it and they have access to anything they may want or need. Please know we are working hard to get to the other side of this Foreign Pandemic, and know we appreciate your continued prayers, patience and support

Everything has stabilized and we are continuing to work hard and stay focused to eliminate Covid 19 from our community. We are succeeding. Activities is doing Face Time for residents and taking them outside when weather permits. Staff are hanging in there and we are recognizing them for being the hero’s they are by facing Covid-19 head on and doing all they can to provide quality care for our residents. We keep our community in our thoughts and prayers.
We continue to improve with more residents and staff recovered from day to day. Because of this, its been a smooth day and we have stayed focused on providing Activities to our residents as best we can, considering some of the hallways are on isolation and us avoiding communal gatherings to ensure we stay socially distanced. The residents spirits are doing well overall.
As of today we don’t have any new staff or residents with positive tests. More great news for now. Following the fundamental CDC guidelines around the clock is what is noticeably reducing our numbers, i.e., screening everyone before they enter the building, wearing masks, hand washing, and using the proper PPE, sanitizing items, floors, handrails, equipment, etc., We haven’t been taking admissions for some time now, as a part of our Infection Prevention measures. We pray that our numbers continue to subside, and that we will soon be Covid free. That is what we are working towards, and that is our goal.
We have had another smooth day. Unfortunately, we have 2 more positive resident test results back. These two, like all other residents, are under our medical director and nurse practitioner’s care. We have medical protocols at the ready in case someone becomes positive, so depending upon their acuity level, we immediately have a doctor’s set of orders to implement and follow. We have Zero new staff results. Everyone in the building has what they want and need. We continue to receive PPE from the company and are 100% stocked.

A fairly smooth weekend. The building has stabilized the Covid spread and many have recovered all together whether it be staff or residents. There are no guarantees with this Chinese Virus, but from great efforts on the staff’s part and a lot of support from God and our company, we appear to be on the other side of being hit by Covid. We do have one new staff case to report, but no new resident cases at this time. We have some more test results coming in tomorrow and we will report those when we have them. Our Director of Nursing and her team have done a stellar job of following the CDC guidelines and our company’s Infection Prevention protocols. It appears the majority of positive residents have recovered, and the facility wide staff testing from last week shows only one positive employee! That is measured Progress! Only a handful of residents, who are isolated, remain from the original group of resident positives. We feel we are on the other side of the terrible Chinese Virus that has caused this World Wide Pandemic.

The majority of our building residents and staff have recovered, and we are retesting the original residents to see their status but we feel most will be negative.
Despite the progress, we are still dedicated to following CDC, health department , and company guidelines to ensure we continue to mitigate spread and defeat Covid-19 in Smith County Health and Rehabilitation.
We continue to receive PPE of every kind, and our company ensures we have every thing we need. We pray all of our staff and residents to have health and no Covid-19. We appreciate the support from our families and people in our community.
Following the CDC and Health Dept guidelines; We are blessed to be able to announce that we have NO resident cases of Covid-19 in our facility at this time. This is a huge milestone that this team has fought hard to achieve. We want to thank ClearView for their constant dedicated support by providing anything we needed to navigate our way through the Chinese Virus; and to ensure our residents have the best care possible. The state of TN and Health Department have also been very supportive and we thank them as well. They have provided everything from PPE to clinical best practices.
Update-We had one resident and 2 staff test Covid positive. We continue to test residents that may present symptoms. We continue to test the staff weekly. We are dedicated to following the CDC and Health department guidelines and have all the resources and support we need.
All of our staff were tested last Thursday and we have NO new positive staff cases for the first time since the testing started!!! Thank You to God first, and second, thank you to ClearView for all their support and resources throughout this ordeal. They go above and beyond to support the residents and staff of Smith County. Third, thank you to the state and the Ford company for sending us PPE.
We received a rapid covid testing machine from the white-house task force. This will not be used for mass testing, but will help us keep one step ahead of covid by catching cases as fast as possible and organizing accordingly.
The facility is doing well and we are focused on the CDC and company guidelines that have brought us through the adversity thus far. No new cases to report at this time.
Since the majority of residents have recovered, and we only have one active resident case, we are converting some halls as non Covid Units. The is encouraging to staff and residents because it is measured tangible progress. We do have a few staff and residents that test positive, but it has been over the 10 day mark that the regulations consider them recovered.
This is no time; however, to drop our guard, and we are not. We are running a tight ship and using our rapid test for any resident who may be symptomatic. This allows us to quickly diagnose, and relocate them if necessary. Many times the symptoms may not be Covid related because people still get other colds or viruses that are not Covid-19.

More good news. We have opened some halls that were closed in isolation due to Covid. The halls are now negative and Open. This lifts the spirits of staff and residents because its proof that we are becoming Covid free. No new positive resident tests to report, and no new staff positive tests to report. We continue to have 110% support from our company, the state and federal government as well as the Ford motor company who graciously sends us PPE.
We are testing the staff now 2 times per week. The last testing still showed zero positive staff. We now have zero positive residents, too. The guidelines for the state say that if our county has over a 5% positive rate we have to test staff 2 x per week, and Smith County is at 11% of its county wide population. We still have 110% support from our company and many others. We want to thank everyone who has helped us to get to this point where we have zero positive cases. We will keep updating our site. Please spread the word that we have been blessed and are Covid free at the moment.

Our county wide (Smith County) positive Covid rate has gone down to below 5% (about 4.5%)
Thus, we will be testing staff 1 time per week. We currently have no Covid positive staff or residents in our building. We will continue to work hard every day to increase the odds of keeping it out. Please continue to pray for our Smith County family.
Our staff weekly testing once again has resulted no Covid cases, so we are thanking God again for another blessed week at Smith County. We still have no Covid resident cases at this time either. We continue to educate the staff on Infection Prevention and we have more than 4 deficiency free State Covid visits in the last 90 days. The staff here are true dedicated heros to the residents. Sacrificing much of themselves for the good of our family. We Thank them for every bit of their hard work.
We still have all the PPE resources one could ask for, and total support from our company and the community.
Still no new cases of Covid positive residents or staff. WE are opening another Hallway soon. We are still focused on following the CDC guidelines to protect our community from the China Virus; Covid 19.

WE still have no new cases of Covid-19; staff or resident related. This is great news. We will be having outdoor family visitation soon. We will likely set up appointments for this later this week.
We are still Covid free in the building , but the challenge now are the state regs stating that if Smith COUNTY’s overall Covid rate is over 5% then we can’t have visitations. The County rate is now above 10% so we have had to cancel our scheduled visitations. We apologize for the inconvenience. We look forward to the day that our lives are back to normal and not dictated by this Chinese Virus.

Smith County tested all residents and all results were negative. The staff that tested positive are not in the facility. We are dedicated to practicing the CDC guidelines to mitigate spread. We have 100 percent of PPE.

Smith County residents are negative at this time. We do however have one staff member who tested positive on our facility wide testing done on October 6th. This staff member will not return to work for a minimum of 10 days, and we are testing our residents today.

We currently have no positive staff or residents in the building. The staff that were positive are now past their 10 days, thus negative. We continue to test 2 times per week with staff and update the results as we get them. We have 100% PPE and have all the support possible from the state, the Feds, our company, and the private sector.
Thank you Lord, we still have no Covid cases with staff or residents. Our PPE stock pile is large and we have 100% support from local and federal agencies as well as our corporate team. No Covid for over 22 days in the building! Our county Covid rate is currently at approximately 15% so we are testing staff 2 times per week. We will continue to post updates. Thanks for following.
